My daughter just started 6th grade (middle school). All sorts of forms were sent home for parents to fill out, including medical info.
I made a note to send in with the form that said that my child, name, is not to receive any type of shot or vaccination without the permission of her father, name.
This should, atleast, inform them that I am opposed to any type of vaccination. Hopefully they will be more inclined to contact me regarding this. Thoughts?
Quoting: TheFiremanGood idea...I was worried about that also. I read and re-read the forms last before I signed any of them. I didn't see anything that could be used in the case of an injection. But I am going to talk with the school nurse about it this coming week just to make sure....
