H1N1 has not mutated since the first strain left Mexico earlier this year. The panic needs to STOP. Keep your hands clean. If you have flu symptoms like fever, non-mucus cough, body aches, chills, moderately tired, chest discomfort and headache, drink plenty of fluids. If you are afraid you have H1N1, go to the hospital for a swab and get treated with Tamiflu within 24-48 hours or it is useless. BE AN ADULT and take care of yourself when you don't feel well and everyone will be fine.

Pandemic (H1N1) 2009, Ukraine - update 1
3 November 2009 -- According to the Ministry of Health of Ukraine, the country has now recorded more than 250 000 cases of influenza-like illness, with 235 patients requiring intensive care. As of 2 November, 70 deaths from acute respiratory illness have been reported.

Regions in western Ukraine continue to show the highest rates of acute respiratory illness/influenza-like illness. The level of activity in the Kyiv area is also increasing rapidly.

Laboratory testing in Ukraine has confirmed pandemic H1N1 influenza virus in samples taken from patients in two of the most affected regions. As the pandemic virus has rapidly become the dominant influenza strain worldwide, it can be assumed that most cases of influenza in Ukraine are caused by the H1N1 virus.

As elsewhere, WHO strongly recommends early treatment with the antiviral drugs, oseltamivir or zanamivir, for patients who meet treatment criteria, even in the absence of a positive laboratory test confirming H1N1 infection.

At the request of the government, a multi-disciplinary team of nine experts has been deployed by WHO and arrived in Kyiv yesterday evening. Discussions with the Minister of Health were held this morning to brief the team.

Team members will now begin field investigations to characterize the clinical and epidemiological features of the outbreak. Work will initially begin in Lviv region, where reported numbers of cases showing severe manifestations of acute respiratory illness have been especially high. Two virologists on the team have started working at the National Influenza Centre and the laboratories of the Central Sanitary and Epidemiological Station in Kyiv to provide diagnostic support.

Samples sent by the Ministry of Health were received today by the WHO Collaborating Centre for Reference and Research on Influenza, Mill Hill in London, UK. The laboratory will conduct confirmatory tests and further characterize the virus.

Many questions remain to be answered. The outbreak in Ukraine may be indicative of how the virus can behave in the northern hemisphere during the winter season, particularly in health care settings typically found in Eastern Europe.

Given the potential significance of this outbreak as an early warning signal, WHO commends the government of Ukraine for its transparent reporting and open sharing of samples.

What I have just heard on the news is that all the victims have tested negative for swine flu. That is why Pneumonic plague has been suggested

The symptoms are the same as SF and Pneumonic plague. Temp, aches and pains and blood in the lungs.

The ECDC, CDC and WHO have sent teams out to Ukraine to see what is going on. This may be Pneumonic plague but it could well be a mutation of H1N1 and if it is we should expect this mutated strain flu to be hitting all other countries in the coming months. The fatality rate is high at 84 in 5 days.

Oh and the vax may well be useless but we are advised to get it anyway.

I just talked to a friend from the Ukraine.

He said that the entire family was sick and they were worried for the kids, but it appears they are getting better. Their grandmother is a different story and they do not think she will make it.

EVERYTHING is closed and most are not working nor really going out.

Tamiflu is not available, nor are surgical masks (Well, they are, but VERY expensive.)

I commend your paper for exposing a panic-causing politicization of a flu outbreak in Ukraine and its negative effect on the election campaign [Tough on Swine Flu, Nov. 2]. The government-imposed ban on political and other mass gatherings, closing of schools and universities for three weeks, a deliberate inflation of a potential health risks and the severity of the flu spread, and a policy to make every Ukrainian wear masks are just recent illustrations of a populist and opportunist character of Prime Minister Yulia Tymoshenko.

In addition to an unwarranted credit for saving Ukraine from the disease, the flu scare creates additional opportunities for personal appropriation of large-scale emergency funding by government officials. Corruption reached epidemic proportions in Ukraine.

However, your editorial fails to note that President Viktor Yushchenko and Viktor Yanukovych, a leading presidential candidate from the Party of Regions, are not less eager than Tymoshenko to embrace such undemocratic policies and exploit the flu and public fear for their advantage during the presidential campaign. President Yushchenko declared a flu outbreak a threat to national security and requested help from NATO. Yushchenko might use it as a pretext to impose a state of emergency and postpone the elections because he is badly lagging in the polls. Viktor Yanukovych called for extending a quarantine to all regions of Ukraine, even though the flu outbreak is currently limited to Western regions. Inna Bohoslovska, another presidential candidate and until recently a leading member of the Party of Regions, in a popular program broadcast by a television channel, which is owned by an associate of Yanukovych, stated without any scientific evidence that Ukraine is experiencing an epidemic of pneumonic plague. Since the first round of the presidential elections is two and a half months away, one has to wonder what kind of other methods the presidential contenders would employ to gain the power.

Primary pneumonic plague results from inhalation of fine infective droplets and can be transmitted from human to human without involvement of fleas or animals.

Pneumonic Plague is not exclusively vector-borne like bubonic plague; instead it can be spread from person to person.

Twenty-eight swine flu cases confirmed in Ukraine, 13 die of swine flu, says emergencies ministry

Twenty-eight cases of the A/H1N1 flu virus have been confirmed in Ukraine, according to Oleksiy Hromazin, the director of the Ukrainian Emergencies Ministry's department for healthcare and medical and biological protection.

He said on Friday that of the 28 confirmed swine flu cases, 13 people had died. He also said that 109 people had already died of flu and acute respiratory infections in Ukraine since the start of the epidemic.

Clusters of Hemorrhagic H1N1 Pneumonia in Ukraine

All the six dead young people had symptoms of severe hemmorhagic pneumonia. The disease starts slowly, with temperature rising to 37.2 - 37.3 degrees, slight cough and pain in joints. Nasal catarrh developed at the end of the second or third day. Autopsy revealed that the lungs were soaked with blood, the oblast chief specialist said.

The above comments are from an early story describing cases in Ukraine. The clustering of hemorrhagic cases raised concerns. The concern was increased by anecdotal reports citing a high frequency of such cases in Lviv (see map). The recent WHO announcement that they were initially focusing of severe acute respiratory illness in Lviv also increased concerns.

Hemorrhagic pneumonia was also observed in the 1918 pandemic and was thought to be linked to cytokine storm. Consequently, those with robust immune systems (previously health young adults) disproportionately died, which has also been seen in the current outbreak (Mexico, US, and worldwide). However, the cases in Ukraine appear to be clustered, raising concerns that the virus has changed.

However, the change may be minor, since the current H1N1 has produced the above symptoms in earlier cases. More severe cases may be linked to a higher viral load, which could be linked to minor genetic changes, or simply due to concentration of virus in schools. In the US the spread of the virus has been linked to school openings, which lead to an explosion of cases and subsequent student deaths. However, now there are increases in fatalities in teachers and administrators, increasing concerns worldwide.

In Ukraine, weather changes and heating issues may have led to a surge in cases, and increased concentrations of virus could have produced conditions favoring high viral loads and increased cytokine storms.

Samples have been sent to Mill Hill, and sequence data should be released shortly. Similarly, investigations on hemorrhagic pneumonia in lviv should help resolve the mechanism for the high concentration of such cases in western Ukraine.

However, H1N1 spread in Kiev has been noted, and other countries such as Belarus have announced additional H1N1 suspect or confirmed fatalities.

Sequence analysis of these cases and an update on the frequency of hemorrhagic pneumonia in Ukraine and worldwide would be useful.
