The body is unified and shared between birth which is why males have nipples; the sex organs grow in differently; so its possible to feel like trapped in the body of the wrong gender; physiologically. In the not so distant past (And still presently in some countries) people like this were put to death, Unfortunate because it is a valid physiological and psychological issue.

If we want to get along with "Aliens", when they come; transgenders should be of no concern. And I am sure any sufficiently evolved race would understand this, Provided they could understand us and communicate.
