The mastoid is located just behind the outside ear. (it is part of your skull). Bone is hard, very hard, and pretty much impervious to chemicals and other stuff 'bleeding through'. thus taking an antibiotic means that while your body is infused with that antibiotic, its going to be real hard for that antibiotic to get into the bone at a high enough dosage to have an effect on the infection. I have no idea what antibiotics you were on. However a bone infection requires a heavy duty broad spectrum antibiotic (unless they took a sample and know exactly which bacteria you have) and a much longer course than a soft tissues infection. Thus we are looking at 6 week course of something like Cipro. Not your usual two weeks. The ear infections and the lymph nodes are all - most likley - related to the mastioditis. That infection of the bone is seeping over into the ears. The lymph-nodes are the trash cans of the human body, its fairly easy to fill them up and even push them past capacity, thus many infections lead to swelling of these nodes, usually the ones closest to the infection. Mind if antibiotics are not working you need surgery to open up the bone to allow for drainage. They drill a slight hole in your head - don't worry it won't go all the way through the skull. Another procedure (a tad painful) is to directly inject antibiotics into that bone. This meets with some success, if you can get your patient to lay there calming with 3-6 injections over the course of two weeks. Since most people don't sit still (or come back - ever after a needle through a bone) surgery is performed. Drilling a hole or a series of holes that allow medicines to quickly go from blood to the infected area. If left untreated you can end up deaf in one ear, or worse, with meningitis as the infection will (eventually) break through the blood/brain barrier.
