I have not found anything yet about the cause of the accident. It should go up on the NRC event notification page sometime tomorrow. But sabotage is always a concern. Sounds like contract negotiations have been ongoing since at least August, so you have workers there who aren't as experienced handling these jobs.

The plant has a long history of problems and leaks.
