Congo-Kinshasa: DRC Ebola Outbreak Expands to Large City as Beni Cases Rise

In a pair of worrying developments in the Democratic Republic of the Congo (DRC) Ebola outbreak, a patient with the virus died in Butembo, a city of nearly 1 million people, and more cases were reported in Beni, an emerging hot spot where community resistance and violent incidents have recently been reported.

On Twitter today, Peter Salama, MD, director-general of emergency preparedness with the World Health Organization (WHO), said the patient who died in Butembo was from Beni. He said the case increases the risk of spread and added that having Ebola in an urban center makes ending the outbreak much harder.

Butembo, North Kivu province's second largest city, is a commercial center and transportation hub with an airport that is not far from the border with Uganda.

Salama added, however, that the good news is the case was detected quickly and the response is already in place and expanding.
Beni cases amid community resistance

Three more Ebola cases have been confirmed, two from Beni and one from Mabalako, the DRC's health ministry said yesterday in its update. Also, one more probable case was reported in Beni, which involved a person who was buried before samples were taken. The developments push the total number of cases to 124, which includes 93 confirmed and 31 probable cases.

Also, health officials are investigating 16 suspected cases. Four more deaths were reported, lifting the outbreak's fatality total to 85.

According to the health ministry, Ndindi district in Beni has become the main focus of the outbreak and is where 70% of the last 20 confirmed cases were reported. It said the cases are occurring amid resistance among the community to working with health authorities. For example, some families are hiding sick people, preventing health providers from taking patients to Ebola treatment centers, and refusing to be vaccinated.

The health ministry in the Democratic Republic of Congo said on Wednesday it had recorded its first case of Ebola in the eastern trading hub of Butembo, a city of close to a million with strong trade links to neighbouring Uganda.

The current Ebola outbreak is believed to have killed 85 people since July and infected another 39. Most have been in villages but about 20 cases have been in Beni, a city of several hundred thousand people with close links to Uganda.

But Butembo, about 55 kilometres (35 miles) away, is two or three times the size of Beni. It straddles a major trading route for consumer goods entering Congo from East Africa, and for Congolese exports of artisanal gold, coltan, timber and other goods to East African ports via Uganda.

Two people died of Ebola haemorrhagic fever in the town of Butembo, opening another outbreak of infection in eastern DRC.

A total of 195 people were identified as having been in contact with these two patients. Patients who must now be vaccinated urgently, but in the area, the fight against the epidemic is complicated because of the numerous population displacements and security problems.

The first Ebola victim in Butembo town was known to the Congolese health services. This is a woman identified as carrying the disease in Beni, but who refused to be vaccinated and moved to Butembo thus opening another focus of contamination.

As proof, the nurse who treated her is the second confirmed Ebola case in Butembo, who died too. 195 people were identified by the health services as having been in contact with these two people. They must be vaccinated urgently according to Dr. Ndjoloklo Tambwe Bathé, sent by the Ministry to organize the response.

• A total of 129 cases of haemorrhagic fever were reported in the region, including 98 confirmed and 31 probable.

• 7 suspected cases are under investigation.

• 2 new confirmed cases, including 1 in Beni and 1 in Masereka. The confirmed case of Masereka is a health worker who is a known contact of Beni but who refused follow-up and vaccination.

Ten people were tested for a battery of respiratory viruses and bacteria in hopes of ruling out serious pathogens that could present a public health threat.

Two of them tested positive for an especially virulent type of influenza A virus, and one of the two, who was gravely ill with pneumonia, was co-infected with another respiratory virus, Cetron said. A third person tested positive for a cold virus.


All 10 patients with respiratory symptoms tested negative for the Middle East Respiratory Syndrome or MERS, a highly infectious and deadly respiratory infection that was first identified in the Middle East in 2012.

Comment: 10 were tested. All 10 neg for MERS. If memory serves, MERS can false positive up to several days after symptoms appear. I have to dust off that portion of my brain, I guess.

Summer may still be lingering, but federal health officials are already urging people to start prepping for another potentially nasty flu season after last year's was particularly deadly.

The Centers for Disease Control and Prevention estimates that more than 700,000 people were hospitalized with the flu during last season with epidemic levels of influenza or pneumonia for 16 consecutive weeks. Some 180 children died from the most recent bout of flu. The vaccine isn't perfect, but it offers the best protection against what can be a dangerous illness, the CDC said. About 80 percent of the children who died from the flu last season weren't vaccinated.

Second monkeypox case recorded in England: report

A second case of monkeypox was reportedly discovered in England, just days after the first-ever case in the country was confirmed in Cornwall.

Both patients are believed to have recently visited Nigeria, but the two cases appear unrelated, the Independent reported.

Monkeypox is a rare disease that does not spread easily between people. It’s considered mild, and typically occurs in remote parts of central and west Africa, according to the World Health Organization. The virus lives in animals but can be transmitted to humans. In 2017 Nigeria experienced its first outbreak of the viral illness since 1978, according to WHO.

“It is likely that monkeypox continues to circulate in Nigeria and could therefore affect travelers who are returning from this part of the world, however, it is very unusual to see two cases in such a relatively short space of time,” Dr. Nick Phin, deputy director of Public Health England’s National Infection Service, said, according the Independent.

While new measures to tackle the Ebola outbreak in the Democratic Republic of the Congo (DRC) are working, the epidemic remains dangerous and unpredictable, the United Nations said on Thursday.

In a joint mission to the epicentre of the outbreak, around the city of Beni close to the Ugandan border, Tedros Adhanom Ghebreyesus, Director General of the World Health Organization (WHO) and UN Peacekeeping chief Jean-Pierre Lacroix, spent Wednesday seeing conditions on the ground for themselves, along with Minister of Health Oly Ilunga Kalenga. They met local officials, health workers, civil society leaders, and peacekeepers.

Since the outbreak began in August, there have been 308 cases and 191 deaths, about half of which have been in Beni, which has a population of around 800,000.

Ebola outbreak now DRC's largest ever, with 319 cases

Officials today reported 7 new Ebola cases in the Democratic Republic of the Congo (DRC), raising the outbreak total to 319 confirmed or probable cases, making the outbreak in North Kivu and Ituri provinces the country's biggest in its history.

"On Friday, November 9, 2018, the Ebola Virus Disease epidemic in the provinces of North Kivu and Ituri just exceeded that of the first epidemic recorded in 1976 in Yambuku, the province of Ecuador," said Minister of Health Oly Ilunga Kalenga, MD, in a DRC health ministry update.

"No other epidemic in the world has been as complex as the one we are currently experiencing," Kalenga added. "Since their arrival in the region, the response teams have faced threats, physical assaults, repeated destruction of their equipment, and kidnapping. Two of our colleagues in the Rapid Response Medical Unit even lost their lives in an attack."

Even with the extensive use of Merck's Ebola vaccine, the virus has held the region captive as response efforts have been slowed by violent skirmishes and community resistance over the last 3 months.

Seven new deaths, 52 suspected cases

In total, 198 people have died during this outbreak, which is 7 more than the DRC reported yesterday. And 52 suspected cases are still under investigation, up from 40 yesterday, according to the WHO data.


Comment: This feels different than the last major outbreak in Nigeria. This seems more like a slow burn, and not a wildfire. IMO, that makes this more dangerous than the Nigerian outbreak.

Ebola response suffers another setback, as WHO evacuates some staff after attack

The World Health Organization has evacuated 16 people working to contain the Ebola outbreak in the Democratic Republic of the Congo following a rebel attack near the area where they were staying, Director General Tedros Adhanom Ghebreyesus said Saturday.

The decision marks another violence-related setback for the response to the outbreak, but Tedros said in an interview with STAT that he hoped containment operations could resume as normal soon.

Overall, the response team in the area comprises more than 200 health workers and others, he said.

The attack appears to have targeted a camp used by United Nations peacekeeping forces in Beni, a city in northeastern Congo that is currently a hot spot for Ebola transmission. The camp is located near a hotel where some Ebola responders are staying.

Tedros said leaders of the response operation called him to inform him an attack was underway on Friday night, and he could hear heavy gunfire over the phone.

“They called me immediately and the gunfire was very close to them,” he said.

As an Ebola outbreak in a conflict-plagued region of the Democratic Republic of the Congo (DRC) continues to spread after 4 months, there’s a glimmer of hope: An experimental Ebola vaccine appears to be helping the communities it reaches. More than 40,000 people have received the vaccine, by far the largest use of it since a trial in 2015 showed it worked well. The vaccine’s effectiveness in this outbreak has not been formally assessed. But Peter Salama, who heads the Ebola response for the World Health Organization (WHO) in Geneva, Switzerland, says, “I think it’s having a major impact.”

WHO, which works in concert with the DRC’s Ministry of Public Health, can’t distribute the vaccine as widely as it would like, however, because of limited supplies, Salama notes. And the obvious targets for vaccination—people who have had contact with cases—have been difficult to identify and reach because of the ongoing conflict; a small number of front-line health care workers have even been caught in the crossfire.

So far the outbreak has tallied some 500 cases, about half of whom have died, according to the DRC’s Ministry of Public Health. It spans a region of the DRC’s northeast that abuts four other countries, and Salama and many others worry about the deadly virus jumping a border, which would require separate response teams and boost the potential for wider spread by infecting people with increased transportation options. Without more financial and personnel support from wealthy countries, the situation could explode into a long-running calamity similar to the Ebola epidemic that devastated three West African countries from 2014 to 2016, warns an editorial published last month in The New England Journal of Medicine (NEJM). A consensus statement from 25 public health and policy experts published the same week in The Journal of the American Medical Association calls the outbreak “exceptionally” dangerous.

Although the toll so far is much smaller than the West African epidemic’s 28,000 cases and 11,000 deaths, it’s now the second largest Ebola outbreak ever documented, and one of the longest running.
